Community Re-Entry Services
The community re-entry service provides the support necessary to allow individuals to continue the rehabilitation process in the real life settings of the home and community.
Three levels of care are offered: Residential Rehabilitation Services, Supported Apartment Living, and Outreach
Goals
- To maximize the independent function of each individual in the context of home and community living
- To enhance each client's quality of life
Residential Rehabilitation
A total rehabilitation environment is offered to those individuals who require extensive physical, functional, cognitive and/or behavioural supports. nrio operates residential homes in Mississauga and Hamilton for individuals who require extended complex care rehabilitation and long term supported living programs. Our Waterloo program offers supported living in an apartment based model. The homes in Mississauga and Hamilton are located in established neighborhoods which are close to medical facilities as well as cultural and recreational opportunities which engage each client in a full range of community living activities. The homes are well equipped for rehabilitation yet they provide the creature comforts of home that are such an important part of community re-entry and supported living. Our staff, residential settings and programs set a standard for achieving an unparalleled level of daily care, quality of life and rehabilitation programs.
Goals
- To provide a structured and supported living environment in which to reacquire skills lost through brain injury, and learn new strategies to improve functional capacity in all areas of rehabilitation, independence and vocational return.
Supported Living
A structured residential environment is offered to those individuals who require extensive physical, functional, cognitive and/or behavioural supports. Individuals who are ready to live on their own are given the opportunity to practice more independent living in the assisted apartment program. The level of supervision decreases as the ability to safely manage independence is demonstrated. nrio supports clients in their own apartments and homes throughout Southern Ontario.
Goals
- To assist the individual with reintegration into the community.
- To enhance independent functioning and self-esteem through support in real life challenges, skill development, and the transfer of rehabilitation gains.
Complex Care Rehabilitation
nrio provides community based rehabilitation services to individuals with complex care needs. nrio 's programs are designed to serve those with severe physical, cognitive and behavioural difficulties that occurred following a brain injury. In the past, these individuals may have been placed in inappropriate long term care settings or remained without services in their homes. nrio has developed programs which respond to the needs of the individual with severe deficits. Our clinic, community outreach, and supported living programs are client focused, outcome oriented, cost effective, and designed to return the individual to their highest possible level of independence.
Residential Program
Rehabilitation services are provided in a home environment to replicate expected future living situations. In this supportive setting each individual is offered the opportunity to relearn and practice functional and essential daily living, community and social skills.
24 hour supervision in a specially adapted home offers a safe, supportive and comfortable environment for long and short-term clients who require close supervision and/or comprehensive rehabilitation programs.
Adult Group Day Programs
Brain Injury Social Recreation Program
As individuals with brain injuries and disabilities leave rehabilitation programs, there is often little available to provide them with ongoing appropriate social and recreational activities or to meet their needs for maintaining the skills gained in rehabilitation. Our social recreation program is designed to provide each participant with a full day of structured and meaningful activities in our clinic and community settings.
Group Stroke Recovery Program
Stroke recovery begins immediately and takes place over a long time, not just during the hospital stay. Our clinical team has created a group based program that provides intense speech- language training, cognitive and physical rehabilitation. In this program we recognize that every stroke patient is unique, therefore every recovery is unique. The small group setting allows for individual attention from our clinical team.
